Forms needed:

Everything is now done on form 605 and associated schedules - both the Ship Station License (Schedule B) and the Restricted Radiotelephone Operators Permit (Schedule E).

You need a separate 605 form for each license along with form 159 – Fee Remittance Advise - one copy to submit with each of the above. On form 605 and 159, instructions are included for most boxes but some are not clear. Please address your questions about the applications to the FCC by calling 1-888-225-5322.

Or if you prefer, we will file out the forms for you, for a fee of $125. License information typically appears in the FCC database the next business day. This is in addition to the required FCC fees. Print out this information sheet, fill in the info,  and fax it to us at 510-769-1573
